Nomination Submission Instructions
Please review the nomination requirements and process as outlined within each award category. Nomination forms are new this year for several of the awards.
Nominations from past years may be resubmitted with updated materials as required. It is highly recommended that nomination forms/letters be dated in the academic year for which the nominee is being considered
Award selection criteria should be explicitly addressed wherever possible in supporting letters or documents.
Note that many award descriptions include a checklist to be used as the Table of Contents for the Nomination Packet.
Nomination packets, including all supporting documentation must be organized in the order listed and emailed as a single PDF file to cahnrexcellence@uconn.edu. Please name the PDF file in the following format: Name of Award_NomineeName
